Session Goals: |
|
This webcast will assist primary care providers to learn how to educate families to reduce childhood exposure to the main indoor asthma triggers. The session will focus on realistic measures to provide such education, as well as look at the disparities in asthma statistics and why children living in poverty have worse outcomes. The Chronic Care Model © will be discussed as it relates to asthma prevention by the health care system. |

Recognize specific issues in medically underserved populations that might be barriers to implementing comprehensive asthma care plans, including cultural, linguistic, economic and professional barriers.

Make recommendations to realistically reduce indoor asthma triggers

Advocate for the use of the chronic care model in asthma trigger management and prevention systems for healthy home environments

Understand the importance of building partnerships between clinicians and patients in the management and prevention of asthma. |
