2. Describe barriers to improved care of people with CKD
3. Describe evidence-based interventions which improve the identification and management of kidney disease in people with diabetes, specifically:
§ Applicability of the chronic care model to CKD
§ Meaning and use of estimated glomerular filtration rate (eGFR) and Urine Albumin-to-Creatinine Ratio (UACR)
§ Optimal blood pressure treatment
§ Screening for co-morbid conditions
4. Describe key concepts of CKD education to enhance patient self-management
5. Describe a strategy for implementing a CKD pilot for improving care in the community health center setting
