Session Goals: |

After attending the webinar, participants should be able to describe the burden of chronic kidney disease (CKD), barriers to improved care of people with CKD, evidence-based interventions which improve the identification and management of kidney disease in people with diabetes, key concepts of CKD education to enhance patient self-management, and they should be able to describe a strategy for implementing a CKD pilot for improving care in the community health center setting. |
