ADOLESCENT/ADULT IMMUNIZATION QUALITY IMPROVEMENT PROJECT

Project Background and Goals:

Clinical Directors Network of Region II, Inc., in collaboration with the Community Health Care Association of New York State (CHCANYS), the New Jersey Primary Care Association (NJPCA), the Utah Clinicians Network (UCN) and the Association for Utah Community Health (AUCH), was recently awarded funding by the Centers for Disease Control and Prevention (CDC) and the Bureau of Primary Health Care (BPHC) to develop and conduct an Adolescent/Adult Immunization Quality Improvement Project. The goal of this project is to develop a methodology for assessing adolescent and adult immunization rates and provider practices in Community/Migrant Health Centers in New York, New Jersey and Utah, and to work with the participating project sites and County, State and City health departments to identify and document issues impacting the delivery of adolescent and adult immunizations in primary care. The methodology and strategies developed through this initiative will be used to formulate an intervention model for enhancing the delivery of adolescent and adult immunizations in primary care.
