Immunizations for Homeless Adults Program

Project Background and Goals:

The Clinical Directors Network, Inc. (CDN) in collaboration with the Bureau of Primary Health Care (BPHC), the Community Health Care Association of New York State (CHCANYS), the New Jersey Primary Care Association (NJPCA), the Utah Clinicians Network (UCN) and the Association for Utah Community Health (AUCH), was recently awarded funding by the National Vaccine Program Office to develop and conduct an expansion to the current Adolescent/Adult Immunization Quality Improvement Project to include homeless adults. The goal of this project is to develop a specific methodology to provide strategies for assessing and improving immunization rates among homeless adults and assessing provider practices in BPHC-supported Community/Migrant/Frontier Health Centers (C/M/FHCs) and Homeless Health Centers in New York, New Jersey, and Utah. The project will also involve working with the participating project sites, national, state and local homeless organizations, and county, state and city health departments to identify and document issues impacting the delivery of immunizations in homeless adults. The methodology and strategies developed through this initiative will be used to formulate an intervention model for enhancing the delivery of adult immunizations to homeless individuals.
