TOGETHER FOR TOTS IMMUNIZATION PROJECT

Project Background and Goals:

In collaboration with the Community Health Care Association of New York State (CHCANYS) and the New Jersey Primary Care Association (NJPCA), Clinical Directors Network Inc. (CDN) implemented the Together for Tots Immunization Project. This project supports the integration of primary care in low income communities by providing assistance in baseline measurement of immunization rates in fifteen (15) selected Community/Migrant Health Centers (C/MHCs) in the New York and New Jersey regions, along with active, ongoing technical assistance, and periodic remeasurement. Through an ongoing collaborative relationship with other primary care associations, health departments and agencies in the region, resources available to the C/MHCs are drawn upon to assist them in reaching the year 2000 goals of 90% immunization of the nation's two-year old children.

The project provides training and support in use of the Center for Disease Control and Prevention's software, CASA (Clinic Assessment Software Application), used in assessing immunization baseline rates, and provides center-specific intervention strategies aimed at measuring and increasing baseline rates.
